In order to process your premium payments smoothly, FMI premiums are paid on a monthly debit order basis. Need to know how to change your banking details? To change your banking details, please complete this form and return it to FMI Client Services via clientcare@fmi.co.za or fax (031) 502 7006. Please note that these changes will only be effective as of the following debit order date from the date requested.
